That reminds me of the novel K-PAX by Gene Brewer (which has also been turned into a motion picture), although it is somewhat different in setup than yours: in this novel, the two different possible explanations are repeatedly considered throughout the whole book (and it may remain slightly frustrating for the reader that the book leaves unanswered which possibility is true).
